PEDIATRIC INTEGRATED HEALTH PROGRAM


 



 

 

What is the Pediatric Integrated Health Program?

It is a FREE service for families with Medicaid benefits who have a child or children with emotional or behavioral health issues. By signing up for this program a team made up of a nurse, care coordinator, and family support specialist will work with you to address all of your childís health care needs, including physical and mental health care.

We know that it can be very confusing and at times frustrating trying to make sure all of your childís needs are being met. Our community has many services and programs available to help families. But it can be very complicated knowing where to start. We are here to help!

Your team will:

  • Be available 24/7 if an emergency happens after office hours.
  • Help you to understand and manage your childís physical and behavioral health services.
  • Provide support and encouragement to you, your child, and family members.
  • Make sure your child has the access to the care he or she needs.
  • Identify and connect you to the community resources needed to make sure your child gets the best care possible.
  • Communicate with all of your childís care providers to ensure everyone is working together for the best results possible.
  • Offer education and preventative care.
This program is offered in addition to any services you are already receiving from Medicaid providers. There is no added fee or hidden charge by signing up for the Pediatric Integrated Health Program offered by Young House Family Services.
